In consideration of the opportunity  to participate in the Wag N’ Walk on Saturday September 27, 2008, and other good and valuable consideration the receipt of which is hereby acknowledged, I hereby knowingly, freely and voluntarily waive and release, on behalf of myself, my heirs, executors, administrators and personal representatives, any action, demand or request for compensation for actual or alleged damages against Animal Rescue Volunteers, Rancho Simi Recreation and Park District, Flood Control, and the City of Simi Valley, their officers, directors, agents, and/or employees and volunteers as result of, arising out of, or in any way in connection with participation in this activity. I understand that this event involves animals and the behavior of animals can be unpredictable and that there are “inherent dangers” in walking and being around animals.

  If you are not 18 years or older, you must have a parent or legal guardian register for you.
